Please fill out our pre-trip check in form below. This enables us to be prepared for your arrival and ensure you have a quality experience. This form must be completed prior to the start of your hunt. If you cannot complete it online, we will be happy to assist you in completing it when you arrive – be sure to bring the pertinent information.

Required Hunting License Documentation (Onsite Purchase)
To obtain a Maine nonresident hunting license, you must provide proof of holding a current or previous hunting license from your home state or a Hunter’s Education/Safety course certificate. We must have physical proof to issue your license onsite when you check in. Please ensure you bring it with you.

Do you currently have or suffered in the past, any of the following:
Outdoor recreation frequently involves hunting & fishing equipment, firearms & ammunition, primitive camp sites and strenuous physical activity. While your guide/instructor maintains Red Cross First Aid certification, ensuring we have a client’s background of any current medical conditions or chronic ailments will help us to help you if an unforeseen incident or injury occurs. We are well aware of HIPA privacy laws and will treat your information accordingly. This is strictly a volunteer questionnaire.
